Admissions Nurse (RN) - Home Health

Adoration Home Health is seeking a Registered Nurse (RN) to join our Home Health team in Fort Wayne, Indiana. As an Admissions Nurse, you will provide comprehensive, patient-centered care in patients' homes, conducting initial assessments and establishing care plans that support patients' clinical needs and personal goals.

Clinical Setting

You will work in home health care, providing direct nursing services to patients in their homes across all counties in the coverage area. This role offers greater work-life balance compared to facility-based settings, with less time on your feet and the opportunity to work independently while maintaining team support.

Responsibilities

• Assess and monitor patients' physical, emotional, and psychological needs
• Create individualized home health care plans aligned with physician orders and patient goals
• Provide direct nursing care including medication administration, treatments, and clinical interventions
• Manage pain and symptoms
• Educate and support patients' families and caregivers
• Collaborate with interdisciplinary care teams
• Maintain accurate and timely clinical documentation
• Participate in on-call rotation as required by the local branch

Schedule

Full-time position, Monday through Friday visits. Flexible scheduling options are available.

Required Qualifications

• Registered Nursing degree (Associate or Bachelor) from an accredited nursing program
• Current unrestricted RN license in Indiana and state of residence
• One year of nursing practice in a patient care setting
• Valid driver's license with acceptable driving record
• Proof of car insurance in accordance with Adoration policy
• Current CPR certification

Preferred Qualifications

• Experience in home health, geriatrics, or related settings
