Admissions Nurse (Licensed Practical Nurse/LPN)

This position is for a weekend admissions nurse at a post-acute care facility. You will deliver quality care to patients during the admission and readmission process, reviewing clinical data for potential admissions and ensuring compliance with all applicable laws, regulations, and facility standards. Occasional per-diem hours are available during the week. Experience in short-term rehab and long-term care is preferred.

Responsibilities

• Provide hands-on assessment care to patients during the admissions process
• Review clinical data for potential admissions
• Evaluate patients' needs assessment and initiate care plans upon patient admission and readmission
• Accurately receive, document, and transcribe physician orders
• Communicate assessment findings and orders to the physician and other team members in a timely manner
• Chart appropriately and timely
• Demonstrate expert knowledge in field of practice and make independent decisions when circumstances warrant
• Maintain professional working relationships with all associates and vendors
• Maintain confidentiality of all proprietary and confidential information
• Exhibit excellent customer service and a positive attitude towards patients
• Assist in the evacuation of patients
• Communicate and function productively on an interdisciplinary team
• Demonstrate dependable, regular attendance

Required Qualifications

• Completion of an accredited LPN program
• Currently licensed and registered as a Licensed Practical/Vocational Nurse (LPN/LVN) in your applicable state
• Active LPN/LVN license in good standing throughout employment
• CPR certification upon hire or obtained during orientation; CPR certification must remain current during employment
• Ability to read, write, speak, and understand the English language
• Ability to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
• Ability to concentrate and use reasoning skills and good judgment

Preferred Qualifications

• One year of geriatric nursing experience
• Experience in short-term rehab and long-term care
