Acute Care Provider (Nurse Practitioner or Physician Assistant)

CommuniCare+OLE, a network of federally-qualified health centers across Napa, Solano, and Yolo Counties, is seeking a Family Nurse Practitioner or Physician Assistant to provide quality primary health care to patients in a team-based setting. You will work at our American Canyon location serving a diverse patient population of over 70,000 individuals, providing comprehensive medical services regardless of insurance status or ability to pay.

Clinical Setting

CommuniCare+OLE operates 17 sites offering medical, dental, behavioral health, substance use treatment, nutrition, optometry, pharmacy, care coordination, and community-based programs. This position is based in American Canyon, CA, with business hours Monday to Friday, 9am to 6pm.

Responsibilities

• Provide assessment, evaluation, diagnosis, treatment, prescribing, planning, and patient education
• Manage acute care, periodic re-evaluations, and chronic disease management
• Review and manage patient information including labs, diagnostic images, prescription refills, consultation notes, and specialist referrals
• Prescribe and administer medications and treatment regimens; review current medications to prevent adverse reactions
• Refer patients to specialists and relevant care services
• Prioritize patient care throughout the day
• Provide acute triage for phone and walk-in patients
• Educate patients on diseases and conditions with appropriate handouts
• Perform in-office procedures as directed
• Document all information in the electronic health record system
• Respond to messages and relay information to patients and external sources
• Direct and coordinate nursing and support staff patient care activities
• Participate in quality assurance, care team development, system optimization, empanelment, and policy development
• Assist in coordination and integration of health clinic outreach programs
• Participate in student education
• Consult with staff physicians as needed

Required Qualifications

Education:

• Master of Science in Nursing (for Nurse Practitioner) or graduate of an accredited Physician Assistant program (for PA)

Licenses and Certifications:

• Current California Family Nurse Practitioner certification (NP) or Current California Physician Assistant License (PA)
• National Commission on the Certification of Physician Assistants Certificate (PA only)
• Current California medication furnishing license
• Current DEA License
• Current Basic Life Support CPR certification

Experience:

• Minimum 3 years of experience required

Skills:

• Strong verbal and written communication skills
• Excellent interpersonal and customer service skills, including trauma-informed de-escalation techniques
• Strong time management skills, ability to prioritize, and ability to work in a fast-paced environment
• Ability to collaborate with providers of treatment in primary care setting
• Knowledge of Electronic Health Record and associated computer programs

Preferred Qualifications

• Experience in urgent care or primary care
• Experience in a community healthcare center
• Bilingual (English/Spanish or other threshold language)

Schedule and Employment

• 0.8 FTE (32 hours per week) or 1.0 FTE available
• Monday to Friday, 9am to 6pm
• Exempt/Salaried position

Compensation and Benefits

Annual Salary: $126,147.84 to $154,169.60 (based on 32-hour work week)
