Acute Care Nurse Practitioner - Hospice, Part-time, Days

Northwestern Medicine seeks an Acute Care Nurse Practitioner to join our Hospice team in an inpatient-focused role. You will provide comprehensive hospice and palliative care services, including face-to-face hospice visits, symptom management, eligibility assessments, inpatient rounding for new GIP admissions, and active participation in interdisciplinary team meetings.

Clinical Setting

This position is 100% inpatient hospice care within the Northwestern Medicine system.

Responsibilities

• Conduct comprehensive health assessments including patient histories and physical examinations
• Provide direct clinical care to assigned patient populations
• Prescribe and order medications, therapies, laboratory tests, imaging, and procedures according to clinical privileges
• Evaluate patients to determine their need for further health care services
• Provide comprehensive teaching to patients and families regarding their care and condition
• Document all care, education, and procedures in the medical record
• Refer patients to other disciplines as appropriate and serve as a consultant in areas of expertise
• Integrate research-based knowledge and clinical expertise in clinical decision-making
• Advocate for patients, families, coworkers, and the community
• Maintain professional appearance, manner, and presentation
• Participate in unit, departmental, divisional, or hospital committees and task forces as appropriate
• Develop and participate in continuing education programs for staff
• Serve as a role model and preceptor to students
• Provide formal education to internal and external groups pertinent to hospice and palliative care
• Participate in quality improvement and research initiatives for the patient population
• Maintain all required certifications and continuing education for licensure and privileging

Schedule

• Monday through Friday, 11:00 AM to 3:00 PM
• On-call duties required: Shared on-call rotation including nights, weekends, and holidays, approximately one week every 13 weeks

Required Qualifications

• Master's degree from an accredited school or college of nursing OR Master's degree from an accredited Physician Assistant Program
• For APRNs: Acute care certification required (inpatient experience as an NP may be considered in lieu of acute care certification)
• For APRNs: First Assist Certification
• Current RN license in the State of Illinois
• Current Advanced Practice Registered Nurse license in the State of Illinois
• Certification as an Advanced Practice Registered Nurse by a national certifying body approved by the Illinois Department of Regulation APRN Board
• OR: Current PA license in the State of Illinois
• Ability to apply for controlled substance license and DEA number
• Privileged and credentialed through the Northwestern Medicine Medical Staff Office
• BLS certification issued by the American Heart Association

Preferred Qualifications

• Prior experience in hospice, palliative care, oncology, or critical care
