1Registered Nurse (RN) Weekend Shift - Home Health

Interim HealthCare is seeking a Registered Nurse (RN) to provide skilled, compassionate home health care to patients in Piedmont, Pelzer, Williamson, Anderson County, and up to West Greenville. In this role, you will deliver direct patient care in home settings, helping patients recover, manage chronic conditions, and live safely and independently while preventing hospitalizations and improving long-term health outcomes.

Schedule

• Full-time weekend shift: Friday 4:30pm through Monday 8:30am
• Service area includes Piedmont, Pelzer, Williamson, Anderson County, and up to West Greenville

Responsibilities

• Provide skilled, patient-centered care in the home while partnering with physicians and the interdisciplinary team to support safe recovery and long-term health
• Perform comprehensive patient assessments, including accurate OASIS documentation
• Develop, implement, and adjust individualized plans of care
• Monitor patient condition and communicate changes promptly to physicians to prevent complications
• Educate patients and caregivers on medications, treatments, disease management, and home safety
• Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care
• Prepare patients and families for safe, successful discharge and document care accurately in the EMR

Required Qualifications

• Graduate of an accredited Registered Nurse (RN) program
• Active South Carolina RN license or Compact License
• Minimum one year of nursing experience

Preferred Qualifications

• Experience in home health, medical-surgical, critical care, or case management
• Familiarity with OASIS documentation
